Specialized Care for Orthopedic Patients

Patients who benefit from the Joint Effort Program include those who have had a total joint replacement, a bone fracture, or other non-surgical orthopedic problem such as sprains or muscle strain. Patients with spinal conditions such as fusions, stenosis, degenerative disc or compression fractures may also qualify for home health services under this program.

The initial visit is completed within 24-48 hours of referral.  One-on-one therapy sessions ensure patient receives thorough treatment. Rehab equipment is brought into the patient’s home.

Physical Therapists incorporate surgeon’s requested protocols. As the physician directs, the therapist will coordinate a therapeutic exercise program, and may apply physical agents such as heat/cold, ultrasound, electrical stimulation or TENS.

Occupational Therapists may provide adaptive equipment, address home safety issues, or suggest home modifications. Working under orders issued by the patient’s own physician, the therapist teaches techniques to simplify the activities of daily of living such as dressing, bathing and home management.

Skilled Nurses follow surgeon’s protocols for incision care, including stitch/staple removal, while monitoring skin integrity and vital signs.  Nurses may provide anticoagulant therapy lab follow-up, and address health issues related to underlying diseases such as arthritis.
