To qualify for Medicare home health services, the patient
must
- Require intermittent skilled care:
nursing, physical therapy, speech language pathology, or occupational
therapy.
- Be homebound. The patient must
require considerable effort and assistance to leave home, and need care on
an intermittent basis. Absences
from home may be attributable to the need to receive medical treatment
including attendance at a psychosocial or medical day treatment program
accredited, licensed or certified by the state.
Patients with a psychiatric disease or condition may be considered
homebound even if their physical ability is unimpaired, when certain
conditions are met.
- Be under the care of a physician
who determines medically necessary and reasonable need for home care, sets
up and reviews a home health plan of care.
