Medicare provides coverage for intermittent home health services for patients with Memory Disorders, including Alzheimer’s and Dementia.  This Medicare home health benefit enables many memory disordered patients to remain safely in their home, with greater ability to function. 

TLC Health Care Services family of companies provides specialized care for the seniors with special needs:  memory disorders, persistent sadness, overwhelming stress, grief & loss.

Under the supervision of the patient’s own physician, TLC Health Care Services provides a specially trained team consisting of

Registered Nurses to manage physical health issues such as incontinence and skin care.
Mental Health Nurse* to address challenging behaviors and work through difficult periods.
Physical Therapist to improve strength and walking skills, and reduce the risk of falling. [link to Fall Risk Program]
Occupational Therapist to promote safety and independence in bathing, dressing and toileting. Also to help the family and caregiver structure daily routines and plan meaningful activities for their loved ones.
Speech Language Pathologist to assist with memory and communication issues, or difficulty swallowing.
Home Health Aide for personal care in conjunction with skilled service.
Medical Social Worker as a planning resource to the family.

To qualify for Medicare home health services, the patient must

  • Require intermittent skilled care: nursing, physical therapy, speech language pathology, or occupational therapy.
  • Be homebound. The patient must require considerable effort and assistance to leave home, and need care on an intermittent basis.  Absences from home may be attributable to the need to receive medical treatment including attendance at a psychosocial or medical day treatment program accredited, licensed or certified by the state.
Patients with a psychiatric disease or condition may be considered homebound even if their physical ability is unimpaired, when certain conditions are met.
  • Be under the care of a physician who determines medically necessary and reasonable need for home care, sets up and reviews a home health plan of care.
